As soon as you are eligible to reapply to reinstate your license, you must complete a Revocation Investigation Packet. After completing the investigation packet, it will be reviewed and determined for eligibility to reinstate, and then Permission to Reapply Notice will be mailed to you. You will also be required to obtain proof of Future Financial Responsibility, which is known as SR-22.
